By registering for a certification program you agree to the following terms and conditions: 

Candidates must be able to provide written proof of training or experience and/or documentation of education completion upon request. Candidates are solely responsible for knowing and understanding state regulatory requirements that may pertain to their certification and/or ability to practice in their profession in their particular state.

Your National Performance Specialists (NPS) certification, like many credentials, will be active for a 2-year period. National Performance Specialists (NPS) certification does not guarantee any candidate’s ability to meet any or all regulatory requirements pertaining to their profession. You must complete CEU credits every 1 or 2 years to keep your certification valid. 

Restrictions are found in California, Louisiana, the State of Washington, and Nevada. Dependent solely upon your employer, as these four states have created state-specific licenses that may be required of you in order to perform phlebotomy. Please check with your employer as to whether these restrictions may apply to you before registering with National Performance Specialists (NPS). National Performance Specialists (NPS) will not be held responsible if you fail to do so, and all applicable cancellation policies and fees will be applied. 

Please read through our Terms and Conditions before submitting your registration. I understand that all assessment questions are the copyrighted property of National Performance Specialists (NPS). The removal or attempt to remove questions or other assessment material from the test site is prohibited. It is forbidden under federal copyright law to copy, reproduce, record, distribute, or disclose these assessment questions by any means, in whole or in part. A violation of this type can result in civil and criminal penalties. 

I certify that I will not cheat or violate the confidentiality of the examination. Cheating or violation of confidentiality may be defined as, but not necessarily limited to the following:

a. obtaining help from any other person during the examination 

b. communicating with or giving help to another candidate during the examination 

c. using notes, books, or any other sources of information during the examination 

d. using electronic programmable devices, such as calculators, cell phones, and PDAs during the examination 

e. reproducing or making copies of a National Performance Specialists (NPS) examination or test questions by any means 

f. memorizing and reproducing test questions 

g. soliciting test questions from previous candidates 

h. discussing or disclosing the contents of the examination by any means or

i. providing false or purposely misleading information when applying for, registering for, or completing the exam. 

NPS reserves the right to adjust certification programs as needed. Changes include but are not limited to Certification program qualifications, inclusion of preparatory materials, certification exam attempts, practice exam access, certification exam passing scores, certification delivery, certification verification methods, certification program costs, and certification material access periods. 

I understand NPS Certified Members are expected to conduct themselves as ethical, medical professionals at all times. NPS reserves the right to revoke or restrict certification services to any user regardless of circumstances. Once services are revoked, a user may never again be permitted to use the services provided by NPS regardless of time passed.

I agree that in the event of a chargeback, also known as a dispute, NPS will place a block on my account for 7 calendar days to allow for my dispute to be withdrawn.

I agree that if I have disputed my payment to NPS and have not resolved or withdrawn the dispute within 7 days any and all NPS certifications held will be revoked and I will not be permitted to take certification exams with NPS in the future.

I understand that my employer may be notified of my NPS certification being revoked. I understand that information about my exam scores will not be shared with anyone without my prior consent.

I understand that I will not be permitted to test or retest with NPS if I have any unresolved disputes. I understand that should my certification(s) be revoked due to an unresolved dispute, I will be required to pay the amount in full, including the dispute fee, and I will be required to retest to regain an NPS certification.

I hereby agree that I will not regain any access to my NPS account in the event of a dispute or chargeback unless I have paid the full amount of the disputed charge as well as the total amount of the additional dispute fee.

I understand that NPS reserves the right to debit my account in the amount of the dispute fee in the event that a dispute is not resolved in favor of NPS.

I understand that National Performance Specialists (NPS) will publish a list of Certified Candidates and that my employer and state regulatory board may be notified if my certification expires or is revoked. 

Unless required by law, my name will not be released to any party in conjunction with my test score without my permission. I understand that any claim I may have related to the good-faith enforcement of these policies or the unintentional damage or loss of my exam records will not exceed the amount of my application fee for this examination. 

 
There is a 365-day access period for all of your certification materials. This means that you can study and take practice exams for as long as you’d like within your 365 days of material access. Once you have passed your final certification exam, your certification will immediately be made available in your account.
 
 
All certification material access expires after a period of 365 days to ensure members are enrolled in the most current version of the certification exam forms. Upon expiration, members are required to repurchase the new materials.
 
 

I understand that this payment for certification materials, course access, and certification is non-refundable.

I understand that my identity will be verified by a third-party verification company prior to exam access. I understand that access to certification materials will be made available upon identity verification submission and that exam access will be provided upon identity verification approval. 


Data & Records Retention Policy

The NPS-CGC has established retention and destruction policies along with schedules for specific categories of records having to do with certification activities, in order to ensure legal compliance, while also accomplishing other objectives, such as preserving intellectual property and cost management.

All documents created or received by the certification agency (NPS) will be retained according to the following schedule and/or in compliance with state law; whichever standard is more stringent. The use of the term “documents” in this policy includes all printed and electronic materials. In accordance with accreditation standards, these records cannot be requested to be deleted. 

Documents received in hard copy may be converted to electronic format for storage. All confidential materials will be retained in compliance with the NPS-CGC Security Policy.

Documents Retained Indefinitely:

  • Examination results (scores)
  • Active applicant/candidate data
  • Active certificant data having to do with recertification.
  • Active and actionable applicant/candidate/certificant data having to do with appeals (exam eligibility, exam performance and recertification), complaints, misconduct, ethical issues and cheating on the exams.
  • Files of active certificants
  • All NPS member accounts
 

Documents Retained for 7 Years Minimum

  • Inactive certificant files (retained for 7 years after file becomes inactive; current active files are retained indefinitely)
 

Documents Retained for 2 Years Minimum

  • General correspondence
  • Routine candidate/certificant correspondence
  • NPS member account activity logs
 

Storage and Protection of Records
All candidate and certificant records are stored and maintained in a secure manner and retained as indicated in the above policy.

Cancellation

You can cancel any time by texting “STOP”. After you send the SMS message “STOP”, we will send you a message to confirm that you have been unsubscribed and no more messages will be sent. If you would like to receive messages from National Performance Specialists (NPS) again, just sign up as you did the first time and National Performance Specialists (NPS) will start sending messages to you again.

Transfer of Number
